Meanwhile in West Virginia, a woman goes to her husband Jackson's mistress's house. Jackson tries to sneak out of the house but steps in front of a speeding truck.

Philip is brought to Three Rivers. Andy tells the team that Rena is his wife. Andy tells Phillip that Lisa is going to take good care of him. Another doctor comes in and says Phillip is having a heart attack.

Later, Rena tells Andy that Phillip's fiance Rebecca is on the way. Andy asks her if she wants to go to couples councilling and she says s he will think about it.

The doctors meets with a stubborn man named Mr. Boyle. Boyle tells Miranda her father treated him well. David examines Boyle and says his liver is not doing well.

Back in West Virginia, a doctor tells Jackson's wife her husband is brain dead. He says Jackson is a organ donor and his liver can be transplanted. After the wife signs the paperwork, she visits her husband in his room. She tells him even though he was unfaithful, she wishes he was still alive.

Ryan learns that there is a liver available for Mr. Boyle. The staff rejoices that they don't have to treat Boyle anymore. Miranda is instructed to pick up the organ with Ryan in West Virginia. Sophia orders David to stay at the hospital to perform Mr. Boyle's pre-op.

Lisa and Andy discover that Phillip might have an aortic anurysm. They try to stop the doctor to stop operating to treat a heart attack, but the doctor says he already injected Phillip with anticoagulants. After the doctor performs a fluroscope, they figure they need to operate, but can't immediately since he was given anticoagulant. Andy tells Rebeca about her fiance's condition. He says he wants to perform a new technique called an endovascular stent graft.

Sophia informs Mr. Boyle he will have a new liver soon. She says David is going to take care of his pre-op. Mr. Boyle says he doesn't want Yo-Yo ma to work on him. Sophia says they are at a teaching hospital and assures him that David will be supervised.

Andy tells Rebecca she only has a minute before they have to take Phillip to the operating room. Rebecca tells her fiance he has to stay alive for their kids sake. After Philip has labored breathing, Andy says they need to go.
