Sam informs Addison that he was able to sew in the tube. Addison says labor is progressing rapidly. Sam takes off to find help.

Addison informs Kara that her baby's foot is coming out the cervix. She explains since the baby is not fully engaged, she is going to push him back in. Jake encourages Kara to keep on going. He says he is now in charge and she has to do this. Addison is successful in pushing the baby back in.

Violet tells Charlotte that Melissa is crying her eyes out in her office. Charlotte tells Violet she seems like a meddler. Violet says the kid's lives are shattered. She says her patients come there to get healed. She says they listen and respond. Charlotte says that is not what she does.

Dawn goes up to Pete and tells him to make a move on her. Pete suggests she try to get with nice guys. Sheldon arrives and Pete tells him Dawn was looking for him. Dawn asks Sheldon if he would like to get a bite to eat.

Charlotte tells Brendon he will not have any long term damage. Brendon says he is going to divorce Melissa. Charlotte tells Brendon she was married once. She says her marriage was a fairy tale until her husband did something stupid. She says she left him and is now with a new boyfriend. She tells him if he doesn't love Melissa, he should walk away.

Addison tells Kara that if she doesn't listen to her she is going to die and disappoint Jake. Addison orders her to push. The baby comes out but he is not breathing. After Addison performs CPR, she is able to revive him. Addison says that someone should recheck the ultrasound since her son is actually a daughter. They try to tell Jake the news, but he doesn't respond. Addison tries to push open the door, but she is stuck.

A helicopter arrives just before a team finds Addison and Kara. They tell Sam to check on Jake. Sam announces that Jake is still alive. Jake tells Sam to take care of Kara just before he is airlifted. Sam tells Kara that Jake lost a lot of blood but will make it. Kara says she is going to name the baby Addison. After they use the jaws of life to open the door, Kara starts to crash. Addison explains that the pressure of the door tamponized the wound.

Brendon tells Melissa that when he looks at her, he is not ready to walk away. He tells her he loves her. Charlotte tells Melissa to keep off him for a couple days. Violet asks Charlotte if Cooper knows she was married. Charlotte says she was never married.

Addison tries to revive Kara, but Sam says she is gone. After Addison stops, Kara slumps over in her chair. As she cries and screams, she tells Sam to get her out of the car. The rescue team use their saw to cut through the metal and free her. Later at the hospital, Addison carries Kara's baby to Jake. After he asks why the doctors ignored his question about Kara, he realizes that she is gone. While in bed with Cooper, Charlotte says she has something to tell him. The next morning, Sam brings Addison a glass of scotch. Addison asks Sam if he remembers when medicine was simple and bad things never happened. Sam says bad things always happen. Addison asks Sam if he thinks they would ever make it. Sam says he was a geek and she was the hot girl. After Addison says she remembers he was a geek, Sam gives her a kiss. They laugh after they tell each other they are not like their ex's.
