Addison watches skateboarders whil holding on to Lucas. A woman walks by and thinks that Lucas looks just like Addison. After Pete takes his baby back, Addison says she has to go to the hospital.

Sheldon tells Charlotte about his dream about peaches. Cooper joins them in the elevator and says he dreamed about Southern peaches that went rotten.

Addison meets with a patient named Kayla who is a surrogate for a couple named Angie and Simon. Addison tells Kayla that the three babies inside her is too much for her to handle and needs to reduce it to one. Addison tells the parents that if Kayla doesn't have surgery soon, she will die.

William and Naomi argue about Fife's protocol which is highly experimental and on the edge of ethics. Fife suggests he has dinner with Naomi to discuss the procedure.

Cooper treats a girl named Maggie who has a cut on her head. After Maggie turns to the side, her parents Joe and Cyndy say Maggie has a an imaginary friend named Eli. Maggie says she introduced Eli to some friends, but they were rude to him.

Cooper tells Sheldon about Maggie who is an eleven year old girl who just moved to town. Cooper agrees to consult her. Cooper tells Sheldon that he thinks Charlotte is using him, but Sheldon says he thinks Charlotte really likes him.

Addison tells Naomi about the operation she will be forced to perform. Addison says she is not sure she wants to be with Pete and Lucas. They see Sam give Vanessa a kiss just before they are introduced. Vanessa tells Addison that Simon and Angie wanted a second opinion on their babies.

Addison tells Kayla and the others that she doesn't think she should deliver the three babies. Kayla's husband Eddie says they have their other kids to think about.

Sheldon tells Cooper that Kayla is schizophrenic and recommends antipsychotics. He says if they start the drugs early, they have a twenty percent chance of giving Maggie a normal childhood.

Angie tells Addison that she can't have children herself and has already bonded with all three babies. Addison says she has to do what is right for Kayla.

Cooper and Sheldon tell Maggie's parents that she is schizophrenic which is a physiological disorder. Sheldon says Eli is not Maggie's creation. Cooper says Sheldon is the expert in this situation. Joe tells them that he doesn't believe their explanation, then leaves the room.

Pete sees that William is having trouble opening a jar. William asks him if he wants to be part of Fife's study on ALS. Pete figures that William has ALS and asks if Naomi knows. He says she doesn't and wishes he doesn't tell her.

Addison tells Sam that his girlfriend sucks since she is putting Kayla's life in risk. Addison tells him to have Vanessa back off, but Sam says he is going to perform Kayla's surgery.

Vanessa and Addison watch as Sam operates on Kayla. While they bicker, Sam tells them to either be quiet or get out. Sam is able to remove a blood clot in Kayla.
