A owner realizes that the man in his bookstore is James Sidas, a former MIT graduate and genius. After the owner asks the man to sign his book, James says his hand is not working.

At the snack shop hospital, Cuddy pays for House's bagel. House fishes for a Thanksgiving invitation, but Cuddy doesn't offer one. House briefs the team about their patient who has ataxia and anemia. House says he is happy to have his team back. House agrees with Foreman that James has TTP and asks them to confirm it with a blood test. James tells his wife Dara not to worry. Taub wonders why is now working as a courier. James explains after he met Dara, he realized he rather be happy than smart.

Chase tells Foreman that Cameron left to Chicago. Foreman asks him to go out, but he says he has things under control. Taub asks Thirteen if he thinks there is something weird with James. He says people who are super intelligent are usually odd. Thirteen says the TTP is confirmed which Taub figures is too easy since House doesn't pick easy cases. House tells the team to perform a splenectomy.

After looking for him all day, House sees Wilson enter the hospital. Wilson says he had a dentist appointment. House says he wants to break up Cuddy and Lucas. He explains there are only two possible outcomes with them, they could either split or stay together. He asks Wilson if he knows where Cuddy' sister lives since he wants to crash her Thanksgiving dinner.

After the spleen is removed, Chase informs James that he will be fine. After James starts to babble about colors and the ceiling, Chase sees that he is crashing.

House asks the team what looks like TTP, but isn't TTP. House informs the team that he is calling all Julia Cuddy's in the phone book. House orders the team to perform an unstandard tox screen while Taub and Chase search James's home. At the home, Taub asks Chase over for Thanksgiving dinner, but he declines. Taub finds mice droppings and wonders if James has leptosporosis. Chase finds a closet full of journals. Taub wonders why Jimmy stopped working. Chase notices a bottle of booze hidden in an airvent.

Cuddy is surprised that House is helping with the flood of patients in the clinic. Cuddy suspects that he has a agenda to either screw with him or prove he has changed. House says he is dying to know which one is it. Cuddy invites him to dinner. House tells Wilson about his accomplishment, but he warns him what he plans won't end well.

Taub reports that they found vodka in James's house while House tries on ties to wear during dinner. Chase wonders if James has liver failure. House orders Taub and Thirteen to perform a liver biopsy. In his room, James tells Dara he takes a shot after work, but she doesn't even know he drank.

After, House arrives at Cuddy's sister's house, he learns that Cuddy's sister took off to Hawaii for Thanksgiving. Meanwhile, Lucas carves the turkey for Cuddy and her friends at another house.

Taub tells Thirteen that James's liver seems fine. He wonders if his kidney is failing. Thirteen tells him they now have to stay and test him.
