The doctors noticed that Cathy was crashing. Derek came in and asked the doctors to stop since she was dead. He asked who was Cathy's doctor, but no one answered.

Webb calls in Kepner to the meeting room. He tells her that Reed just gave him her credentials. He also believes she is a good doctor. They ask her why Cathy's organs failed, why she needed a chest tube and a crike. Kepner remembers being distracted by the man with the axe in his chest in the middle of examining Cathy's airway. Webb says Kepner never looked in and saw the soot in her throat which should have been immediately treated by intubation. Webb says since Kepner made a mistake that led to Cathy's death, she is fired.

Meanwhile, Lexie talks to the burn victim. Cathy's husband arrives, gives his son a hug then looks for Cathy.

Derek tells Webb they are all good doctors. Webb says he wanted to know who was responsible. Derek says it might not be one doctor, but it might be too many doctors who don't know or trust each other. Derek says since the merger, the system now is in chaos. Derek asks him to look again to find who is responsible.

Alex tries to get a hold of Izzie, fails, then throws his phone into the wall. The firefighter is brought to his room accompanied by his friends. Reed tells Cristina that Kepner was a better doctor than her. The doctors says that they can't believe Kepner missed Cathy's airway, which is the first thing she should have checked. Cristina says they all made mistakes and any one of them could have been fired. Kepner walks out of the hospital without saying a word. Meredith is finally released by Bailey. She tells Derek she is ready to go home then asks what happened that night.
