A man hops in his van and pours a bag of ice in a cooler. He uses alcohol to sterilize a small metal scooper. He parks at an elevated garage and watches two girls enter the building. As the girls climb the stairs, he slashes their throats.

Derek tells Tamara Barnes about his promotion. She admits that she feels better when she is around him. She offers to buy him a drink after his career calms down. She gives him a necklace and explains it used to belong to her brother. She thanks him for being there for her.

The team gossips about Hotch's decision to step down. Derek tells Strauss what case he wants to work on next. Hotch tells Derek that he should use his office and that he is going to clear out soon, but Derek says he doesn't want to take it.

JJ briefs the team about their new case. She says two girls, Megan and Bina, and a man named John were killed and had their eyes gouged out. Spencer says the term for this type of killer is called an enucleator. Derek says he chose to work the case since there is a close proximity of time between the murders. Hotch says enucleators are repeat offenders and will most likely strike again.

The team figures that the killer is delusional and was probably institutionalized. They wonder if the unsub was more opportunistic than planned. JJ tells the team that one of the girl's family asked to speak to the team leader. Derek tells Hotch and Spencer to see if the killer targeted John for personal reasons. Garcia calls Spencer and informs him that there are no OKC suspects who liked to gouge eyes. The Oklahoma detective, Brantley, asks why the killer would want the eyes. Spencer explains the eyes might be trophies, but they break down quickly. He says that some killers like to eat the eyes.

Derek and JJ meet with Bina's older sister. She explains because of her Buddhist religion, they are not to allowed to leave. She tells them that her grandma said she saw Bina's ghost looking for her eyes. The sister says they want to cremate her, but they need all of her. Derek says they will try to find her eyes, but they might not be able to recover them. The sister says Bina might be cursed forever.
