Spencer tells David that he found a pattern that shows the babies the suspects took were dropped off only at Presbyterian churches. He says he thinks they live in an area near three churches. After discovering the names of the babies who were dropped off at the church, David figures that the suspects are only keeping the boys.

Meanwhile in the cell, Carol starts to deliver. Carol tells Julie if she doesn't deliver, the captors are going to kill her.

Derek tells JJ he is stressed. JJ says that Hotch's bosses are making him justify every decision.

A doctor tells David that the wife is not using normal drugs for the delivery. She says the suspects are probably mixing drugs to help carry the babies to term. David wonders what other kinds of diseases the drugs treat.

Carol tells Julie that this is not the first time she is delivered a baby for their captors. She explains that they want boys. Carol asks Julie to help her because it is time for her to push.

Hotch asks Derek to write up a tactical arrest plan. Derek says he never had to do that in the past. Derek says he is sorry that Strauss is on his back. He says he understands how Stauss feels since even he is worried about him. Hotch says he does a lot of Derek's work so he can spend more time finding the unsubs. Hotch says he doesn't have to justify his orders.

The team learns that the female unsub was taking breast cancer drugs. Garcia finds that there is one female named Linda Reimann who had breast cancer, gave birth to a stillborn baby named Michael, and is married to a man named Robert who had a criminal sexual assault record.

Meanwhile, Carol delivers the baby. Robert walks down the stairs, takes the baby and hands it to his wife Linda. After Robert says it's a boy, Linda names him Michael.

Derek shows the team the layout of Robert's house. They figure that Robert built a dungeon. Hotch suggests they employ a soft entry with Emily and JJ leading in case there are children present.

The team arrives at the house. They unlock the door and JJ and Emily walk in with guns drawn. They see a boy on the couch and ask him to come outside. They hear a toilet flush. The FBI surprise Robert and ask him where his wife is. They find Linda in the nursery. She tells Derek she always knew they would come. Derek tells her she should know what she is doing is wrong. After David and Emily find Carol and Julie in the basement, they call for a medic. Linda says all she has is the baby. Derek tells Linda to hand over her baby and go back on her breast cancer treatment. He tells her that Michael will always remember she helped him into the world. Linda kisses Michael on the head and hands him over. Downstairs, Julie asks for her son. Derek brings Michael to her.

Later, JJ tells Monica's parents that Lisa's parents have agreed to give them visitation. She says Monica also had a son. JJ introduces them to Michael, the boy they found on the couch.

Hotch tells Derek that the FBI thinks his leadership has been compromised. Derek says he doesn't question his command. Hotch says he is going to step down so no team members have to go through scrutiny and face reassignment. Hotch says he is not leaving the team, but he is no longer going to be in charge. He tells Derek he will be the new leader in order to keep the team together. Derek figures that is why he was being pushed. Hotch asks Derek if he will take the job. Derek says his promotion is only temporary until they find Foyet. Hotch gives Derek boxes of files they have to go through.
